Forms
Please download the appropriate form(s) by clicking the forms button and thoroughly complete them (using BLACK ink) prior to your appointment. Your complete health history is extremely important in assisting with your evaluation; please take your time with the forms, and use extra paper if you care to provide additional information. In addition to the forms please bring or have faxed (530.342.6234) any recent labs, medical history, your insurance card(s), including your prescription card, your ID, and any bottles (or a list) of prescribed medications you are taking
Monday, Wednesday, Thursday - 9:00 am - 5:00 pm
Tuesday - 11:00 - 7:00 pm
Friday, Saturday & Sunday - CLOSED
